A New Level of Post-Acute Pulmonary Care
The Center for Pulmonary Excellence at Hampton Ridge was developed in conjunction with top pulmonologists and nurses to provide patients with a high level of care and management for a broad range of pulmonary conditions, such as COPD (Chronic Obstructive Pulmonary Disease), pneumonia, and post-tracheotomy, among others. The Center is affiliated with area hospitals, and plays a pivotal role in supporting patient needs and improving clinical outcomes.
Scope of Care
The Center for Pulmonary Excellence features advanced clinical capabilities including the administration of piped-in oxygen with a capacity of 15 liters per minute, enabling clinicians to deliver 100% oxygen to patients as needed. The Center’s care program addresses a wide range of pulmonary needs, with treatment that includes:
- Chest Physiotherapy (CPT)
- Nebulizer/breathing treatments
- Respiratory therapy visits
- Advanced patient and family education
- BIPAP and CPAP
- COPD care
- Pneumonia care
- Tracheotomy care
- Post-ventilator care
- Peak flow management and supervision
The clinical team works in close coordination with each patient’s primary care physician to develop and effectively implement an individualized plan of care and customized exercise program to help patients with their shortness of breath and other pulmonary symptoms.
For COPD patients, the regimen includes exercises to improve strength and stamina, empowering patients to engage in more activities and enjoy an overall enhanced quality of life.
Patient and Family Education
The key to successful pulmonary care and treatment is ensuring a sustained follow-up maintenance plan. The pulmonary team’s skilled instructors train patients and family members in the protocols and tools they need to manage their condition, emphasizing the importance of maintaining their regimens.
COPD patients are provided with a Peak Flow Meter, a Daily Journal to track progress and stability, as well as an individualized COPD Action Plan. The rigorous training and individualized patient tracking tools has dramatically increased patient compliance from the State average of 20% to more than 80%.
